Thanks for all the replies. It was the device driver. I had gone to 12.8 from 13.4, but now rolled back to 11.12 it works.

I just installed FAH. Two slots were set up, one CPU and one GPU. The CPU slot runs fine. The GPU slot waited until I moved the utilization slider to full then tried to start but failed with an I/O error on the hash check. What next?
