Removed, closed, reopened, readded, still core 15 and 0.00%. No special settings for slot or client.

Perhaps you're assuming that slot 0 and slot 1 somehow refers to the physical position on the motherboard. The order that they're detected is not related to anything obvious.
Remove BOTH GPU slots. Add one. Make it work prior to proceeding to the next GPU slot.

Since reinstalling FAH, I have not rearranged the slots (in FAH or literally), the 9500 is working great, just not the TI.

Post the System Info (middle tab in FAHClient) with the 9x00 folding.
Manually add a GPU slot.
Post the new System info.
if it works, great. If not, run FAHBench and copy the preamble shown before it actually starts testing.

Ran through the manual settings and FINALLY got the 750 Ti running at 66K PPD, then the 9500 stopped working.
On the 9500:
Went through FAH Bench 1.2.0 (openCL) and got deserializing, then creating context, followed by:

9500GT is too old to complete the benchmark - it's too old for core17 WUs as well. But "FahBench --display-devices" is able to detect the device, thus providing useful troubleshooting info. If you want to do something useful with 9500GT - in addition to connecting the display to it - do the max-packet-size=small thingy and fold core_15 WUs with it.
Note that not all pre-Fermis are compatible with core15 WUs. For example, my ION iGPU (essentially an embedded 9400GT) failed whereas my discrete 9400GT graphics card worked just fine AFAICT. Go figure. If a pre-Fermi GPU doesn't work with core_15, you'll usually see GPU_MEMTEST_ERROR a minute or two after you've started folding with it.

I *did* in fact start seeing the memtest error and then eventually came up failed. I need dual monitors for work, so maybe I'll buy a nice cheapo card for like $40.
I'm sure I'm in good company when I say... it erks me that one of my cards won't fold, basically just on principle.

Not principle, but time aka mixing old with new. Even if the NV driver you are using happens to support both GPUs, you would run in to trouble the next time the 750 Ti downloaded a core_18 work unit. Maxwell GPUs need a driver v347.xx or newer to work well with core_18.
In that light, my next troubleshooting step would be to remove the 9500, and get the 750 working with the latest drivers.

True. However, the 2nd one could be a budget GPU like a 730 with GDDR5 - low wattage, support fo multiple displays (DVI+HDMI+VGA) and compatible with latest and greatest drivers. I've seen 730s with 2GB GDDR5, in case you think 1GB isn't enough. We'll see how it goes, ordered my adapter, then going to pull out the 9500 and see if I can get $20 or so for it.

FYI for future note, the 9500 GT is running great right now with the 750, both are folding well. Running latest driver right now, 353.06. The 9500 is running 0x15 on "beta" setting, and 750 is getting 0x17, no problem.
The 9500 today is running 33K PPD, though it did take several hours before it got a WU.
So future reference, 750Ti FTW and 9500 GT can co-exist and co-fold on 353.08.

Sorry to quote an old post but how exactly did you do this? I'm on windows 10 though and don't have a driver that supports both at once in windows 10. 33k PPD on a 9500 is completely unrealistic : ~11k was the best I could do on 9800 GTX+ ...
As stated before, running GPUs from different generations is usually a pain, and it often confuses the client on GPU identification.
33k PPD for the 750 Ti sounds a little low, but more coherent.
