You don't need 3-phase for high power levels into US homes - standard 220 volt split-phase is widely available at the 600 amp level (though that's usually for businesses), and I've seen them go as high as 1000 amps on 2 occasions.
More than 200 amps isn't COMMON to residences, but it's getting more so (300 seems to be the new "norm" for higher-end houses) in areas where electric heating is the norm.
220 volts is normally 2 hots + ground, unless it's being used on something that needs BOTH 220v and 110v (like a range with a clock/timer or many driers that use a 110volt "timer" control) in which case the neutral will also be included.
